New Patients
Dr. Stefan is excited to meet you and address your concerns. Our New Patient forms are available below to print and fill out prior to your first appointment. If you are unable to print out the forms, please arrive 15-20 minutes early to fill them out in the office. Please bring your ID, insurance card, and list of medications if applicable.
